Here's what we'll need you to do:

► Complete the Application for Credit form.
► Complete the Bank Inquiry form. Banks now require a signed and dated authorization
from you before they will give us the information needed to open your account.
► Send us your latest Balance Sheet.
► Read our Terms and Conditions in the Help Center on our website.
► FAX the Application for Credit, Bank Inquiry forms and your Balance Sheet to us at
The time for the credit approval process is dependent upon the timeliness of receiving the Bank Inquiry from your bank.
If we need more information, we will contact you.
Once you have been approved for our Net 30 Day terms, Your customer file will be updated.
We require orders to be placed in writing. You may FAX your Purchase Order to 714-524-8145 or you may use our FAX order form found in the Help Center on our website under Shopping Tools. You may phone in your orders but they must be followed up by a FAX.
You may enter your orders directly into our system if you so choose. You can set up your login and password by registering on our website, www.plasticproductsmfg.com. To log in, you would go to our system host, https://system.netsuite.com/pages/login.jsp, and enter your user name and password.
